(4) Combined Declaration and Assignment : The inventors take an oath that they are the original inventors and the inventors assign the rights of the patent to the affiliated corporation or a specified person/group without claiming any reserved rights regarding the application of the patent if issued. (Please seek advice from our firm if the case requires a special form of assignment or joint ownership.)
(5) Power of Attorney : The Applicant appoints the practitioners of our firm as the designated United States attorneys to prosecute this application and transact with U.S. Patent and Trademark Office.
